Local authorities have two legal responsibilities in respect of young people with learning difficulties and/or disabilities. These responsibilities are delivered by Connexions Staffordshire through our contractual arrangements.
Transition Reviews
If a young person has a Statement of Special Educational Needs (SEN) they will have a Transition Review in Year 9. The school will call a meeting to which the young person and their parents are invited, together with representatives from any agencies that are involved in supporting the young person. A Connexions PA will be in attendance and will contribute to the review. The purpose of the Transition Review is to plan the young person’s transition from school to adulthood. The review will provide young people with an opportunity to share their thoughts about the future. A Transition Plan will be written following the meeting. The Transition plan will state:
The Transition Plan will be reviewed each year the young person remains at school.
Section 139A-C Assessment (formerly Section 140 Assessment)
If a young person has a Statement of SEN and they choose to leave school at the end of Year 11 to go to college or onto a work-based learning programme, a Connexions PA will complete a Section 139A-C Assessment with them. A Section 139A-C Assessment may also be completed if the young person is identified as School Action or School Action Plus. In Year 12 and above, assessments may be completed for young people who are School Action, School Action Plus, or who have a Statement of SEN. The assessment will detail a young person’s support needs and the provision required to meet those needs. In order to gather the necessary information, the Connexions PA will liaise with the young person, their parents or carers, the Special Educational Needs Coordinator (SENCO), relevant school staff and relevant specialist agencies. A report is written and, with the consent of the young person or their parents/carers, submitted to the college or work-based learning provider that the young person is going to attend.
The report will provide colleges and training providers with information on the support a young person will need as they begin their course.
