Future Choices – A guide for Parents/Carers of young people with learning difficulties and/or disabilities.  Produced by Connexions Staffordshire, this publication is for use with Parents and Carers and provides information to help with planning for a young person’s transition to adult life.

My Future, My Choices. Produced by Connexions Staffordshire, this workbook is used by young people with learning difficulties and/or disabilities in Year 9, to help them to plan for their future.  Schools in Staffordshire currently receive this publication, but schools in Stoke-on-Trent use alternative Transition Pathway Resources
 
Stoke-on-Trent Parent Partnership Service works with parents and carers of children with special educational needs and encourages partnership working between agencies that support children.

Staffordshire Parent Partnership Service works to develop closer links and better communications between families of children age 0 – 19 with special educational needs and all involved with their child.
